Use this form when enrolling your cadet to provide a record of your son or daughters medical history. This form should be resubmitted annually by all hands or more frequently if there is a significant change in medical history. Adult leaders will resubmit this form prior to acting as Escort Officer for TWT.

Payment of all fees is due upon enrollment. A uniform deposit of $30.00 for League Cadets and $45.00 for Sea Cadets is required from all new recruits. This deposit will be returned when the cadet returns all uniforms in good condition after leaving the program. Effective 01 JAN 07 the Enrollment Fee / ID renewal fee will be $55.00 for all cadets. Dues for all cadets is $5.00 per month and is payable upon enrollment and at the first drill of each month thereafter. Please make all checks out to "NSCC".
